Your AI says “done”. Who checked?

An AI working for you makes claims all day long. Until now, nobody checked them for it.

The finding

An AI does not lie. It is confidently wrong, and it does not know it.

When you ask an AI to publish a page, send an email or run a nightly report, it answers “done”. Most of the time it is true. The rest of the time, the page is cached, the email went to a dead address, the scheduled task never started. Nothing breaks, nothing rings. The AI has no way to know, and you even less.

A 2026 study of 58 agent tasks rated “successful” found only one in five was clean: 83 % hid a procedural error. A developer reported scheduling seven agents, two of which never ran for eighteen days while every dashboard stayed green. We lived it at Synapse: four days of reports never sent, a paid service doing nothing, and an AI answering “scheduled” in good faith.

Agent platforms log everything their agents do. But the agent writes the log. A student marking their own paper always gets 100 %.

A signed log proves the story, not the world

Even serious cryptographic records prove a story was not altered afterwards. They do not prove the page answered, the email could arrive, the task ran.

The giants watch their own AIs

Amazon, Microsoft and Google each attest their agents, in their own house. None vouches for the neighbour’s AI. A third party that belongs to no one is missing.
